sunsilk contest

Sunsilk man AND his charger

She was ugly.. very ugly. Her hair was very dry, fizzy, puzzled.  Nature is beautiful. But disasters like flood, earthquake and many more things are also part of nature. When god made somebody beautiful we praise Him. We say “what a beautiful girl God has created”. What about ugly people? Who made them? God? The only thing exist in this world which speaks truth is mirror. Whenever she used to see mirror she used to smile. Smile on her own ugliness. She was just 18 but she used to count wrinkles on face. Her hair was so curly that she use to cry with tears every time she use to comb. Ok,ok, that’s enough.. It is not a serious story. Actually she was the successor of great, legendry family of witches. She has to look ugly. In her family ugliness was the beauty, ornament of the girl. She was the ugliest girl in the family. Every female in family had some special quality. Her mother had long nails, her aunty had long teeth, and her grandmother had long ears. These all special features further add in their ugliness. But here is the sad part she was very ugly just like great witches but she didn’t have any special quality. Just because of this she never got selection in famous college of witches. She used to sit in front of mirror and used to cry.

“Why I don’t have anything special like my mother?”

Suddenly mirror started speaking

“My dear child. God is great. He didn’t give you any special feature but he gave you courage to do something extra ordinary. Just identify your inner strength and power. There must be something special in you.” (well let me clear here all witches have mirror which speaks  so take it literally )

“Please God help me. Please tell me what I should do?”
She was continuously looking herself in mirror with tears in eyes.

Suddenly another strange thing happened. A man appeared in front of her.
She got bit shocked
“Who are you who scared the witch?”
                                              “ I am Sunsilk Man”
                                               “Who Sunsilk Man?”
“God sent me. You consider me as incarnation of God”
Sunsilk man had strong body, shiny hair and bright eyes. (he doesn’t wear underwear outside).
“What you do Sunsilk Man” she asked with curiosity
“I am God of hair. I make hair shiny, long and strong.”
                                             “why is your name ‘Sunsilk’?”
“ Because I will make your hair long, bright as Sun and shiny like a silk”( real reason is to praise  our sponsor sunsilk )
“My dear child you are very sad and unhappy that u don’t have anything long  so I am giving you one shampoo( Sunsilk shampoo). After using this shampoo u will able to make your hair very long which would be your special quality. With this shampoo I am giving you special Sunsilk Charger too.”
“Special charger? What is it “
“You can recharge your hair by this charger and give current shock to whom you want to scare. It will certainly help you to become great witch”

After this event she started using sunsilk shampoo and charger every day. She got the selection in famous witch training college and got the title of the ugliest witch in college too. This is all because of Sunsilk shampoo and Sunsilk man who came as angel in her life and changed the angle of life (nice line boss).

Now question is how sunsilk worked in my life. Initially when I didn’t know about Sunsilk I used to recharge my hair with mobile charger. After that I used tablet charger and laptop charger too but never got satisfactory results. But now I am using Sunsilk shampoo to recharge my hair and believe me this is the best u can see my hair in photo

Acne - let's discuss everything part-1

DR. GAURANG GUPTA
dermatologist, Cosmetologist

Acne … almost all of us suffer with this problem in young age.  Few get this occasionally, some have very severe form and some get sudden flare up. Let’s start again from prevention. Actually acne is not a disease .. it is a physiological process due to hormonal changes in body but when it creates problems and disfigurement of face treatment becomes necessary. Whenever patients come to me I start with these points which can reduce flare of acne
1st . Don’t use hair oil and any sticky cream on face.
2. avoid oily food , chocolate , milk products
3. avoid dust, use sunscreen( above 30 SPF)
There so many medicines and creams in dermatology which are proven effective for acne but frankly speaking all are just not very effective. Before I prescribe any cream to patient I say one thing 1st that is
1.       Any cream don’t rub it on face
2.       Apply it over acne ..and if u feeling redness, irritation, burning then just put only for 20 minute then wash it. Retinoic acid , benjoyel per oxide, salicylic acid are known for irritation so do it as advised.
Now let’s start with creams. There are many creams and there is specific way to use.
Clindamycine ( clindac A, CLINTOP, FACECLIN, CLEAR GEL) -  this is more of preventive and curative so I tell patients to use it all over face in the morning. Now a days nano gel preparation are coming which penetrate more and more effective too.
Adapalene gel ( adalene, defferine,)– this is my favourite .. I tell to use it over acne in the night.
Benjoyl per oxide – it is effective antibiotic but I personally don’t like this cream because it cause too much red ness and post acne marks. Still it is favourite of many old dermatologists.
Salicylic acid (below 3%) – it is also effective but not as much as adapalene but some patients responds better with this than other drug.
Now a days combinations of above medicines like adapalene+clindamycine
                                                                         BPO+ clindamycine
I give them according to acne type and nature
I don’t go beyond these creams because these are the best medicine can be used in acne although there are other topical drugs for acne but all are costly and less effective than these above things.

About oral medicine, peel, and other things I will write in next post. You can ask your queries here in comments or email me dr.gauranggupta@gmail.com

Isotretinoin part 2

Isotretinoin, a magic drug for acne which works very effectively in severe acne. In previous post I have written about its side effect and uses. In this post I will explain more about this drug. Main complaint by patients is excessive dry ness, dandruff and lip dryness. Many patients leave the course in middle due to excessive dryness. I strongly suggest to my all patients to use moisturiser at least twice a day. I start this drug with dose of 20 mg and if I see less response I increase it up to 60 mg. 2nd important thing is dietary restriction. This drug increase triglecirides in blood so patients shouldn’t take oily, high fat diet during the treatment. Drinking lot of water always helps with any medicine. Use of SUNSCREEN is very necessary because patient may feel bit irritation in sunlight during treatment. Drug compliance means how patient taking this drug is also important. Its absorption is more after meal or with meal. As I mentioned in earlier post that female cannot have pregnancy up to 2 year after taking this medicine.  Excessive Alcohol consumption may also create problem with drug. For further query email me dr.gauranggupta@gmail.com

"Isotretinoin" MAGIC DRUG BUT

Many people discuss, talk about this accutane isotretinoin pills. Even i am very fond of prescribing this drug to acne patients. It is very effective in acne, folliculitis, acneiform eruption. Infect it is a drug of choice for acne. It is not only comedolytic but also it decreases sebum production by oil glands. So it makes skin dry and fair too. Basically it's doses depend on doctors experiences. Many doctors give 20 mg / day for 1-2 months. But doses can go up to 60 mg/day. Main problem with isotretinoin is its side effects. 1st is it affects foetus development so we strictly take written consent not to have pregnancy up to 1 and half year. Its effect remain in the body up to 1-2 year so even after stopping this drug female patients can not conceive for 1 year at least. This drug also increases blood triglyceride and LDL level so i monitor blood triglycerides every month. If we keep these points in mind it is very safe and effective drug for acne.  Let me repeat that i have very good feedback with this drug in acne patients and if doctor is experienced it's side effects can be avoided easily.

Acne Scars BYE BYE

Acne scars is most common cosmetic problem. Acnes damage the skin texture and cause acne scars. There are different types of acne scars. Above picture showing various kind of scars which can be caused by acne. Rolling, box and ice pick scars are common in all of them. There are various method to treat these scars. We have lasers(co2 fractional laser is very good), carbon facial, dermaroller, subcision, chemical peeling, derma abrasion to treat these scars. I very frequently use laser+ dermaroller+subcision combination to treat scars. It takes around 4-5 month to complete all sessions. In 1st sitting i do laser then next sitting is for dermaroller+subcision. We repeat these procedure alternatively. Almost 7-8 sitting are required to get proper improvement. But let me tell you that there is no method to get 100% cure. Even after doing all these advance and costly procedure there is up to 60-70% improvement. My all patients find it very effective and it improves their confidence surely.

Hair Loss and mesotherapy

mesotherapy is a new technique which becomes very popular among doctors. it is very simple but effective way to treat androgenic alopecia. beinng doctor i know that there are very few option to treat androgenic(familial) baldness. one is topical minoxidile and oral finasteride. 2nd is hair transplant. mesotherapy comes between these two method. according to few study it is 400 times more effective than topical minoxidile. in mesotherapy we inject cocktail or mixture of various chemical like growth factor, minoxidile, some herbal and other vitamin, protein etc. these chemical inject below the skin by microneedles. this procedure can be done by normal insulin injections by puncturing the area again and again or there are special mesotherapy guns. there are atleast 6 sittings are required and after two sitting patient starts noticing improvement. there are no side effects except bruising and little pain while procedure due to multiple puncturing of microneedles. main problem with this procedure is patients need to continue sitting to keep hair grow. it is not a permanent cure or treatment for baldness. the only permanent treatment for baldness is hair transplant. mesotherapy can be used for fat loss, wrinkles, bladness, and other cosmetic procedures.

DIWALI and SKIN

Happy Diwali to ALL.

Diwali means lots of sweets and fire cracker. these things can affect your skin and can cause flare up of acne and other skin disease. lets see how.

FIRE CRACKER

this is actually enemy of skin. it contain lot of chemical poisons. its fume and smoke directly react with skin and can cause pigmentation, acne, excessive sweating, sebum production and chemical dermatitis. other than this small-small chemical particle can block the pores of the skin which directly activate hair glands to produce acne. and of course always there is a chance to get burn with fire cracker. i suggest that while enjoying  with fire crackers always keep your face covered with cloth. use clindamycine gel and sunscreen blocker even if there is night. sunscreen although protect skin from sun but it act as blocker too and prevent chemical particle to get settle on skin. 2nd things do not use oily creams over face because particle get stick to oily skin and develop acne.

SWEETS

various recent studies have proved that glucose(sweets) activate IGF(insulin like growth factor) receptors in oil glands. Because of activation of these receptor oil gland start producing lot of sebum and oil. And more oil means more acne. i advise just avoid milk product sweet and chocolate.

HAVE A SAFE DIWALI

What Is Yellow Peel

basically yellow peel is a kind of flavour in curry of other dermatological procedure. this peel contain phytic acid, kojic acid, and azaleic acid mixture. these chemicals cause epidermis and dermis repair, helps in reducing pigmentation, wrinkles, acne, sunburn etc. it also gives instant glow type feeling. it is a mild peel so i always use it with other peel. like we can give this peel after glycolic or salicylic acid peel. this peel can be repeated with other chemical peel simultaneously.
it is a good peel for working people because its effects on skin is almost invisible. this peel works as a healer for skin. it increase and activate repair cells in skin.

use of yellow peel

reduce scarring
reduce pigmentation
reduce wrinkles
reduce roughness

Dabur lal tail contest

                                    Dabur Lal Tail

I, My Mother and Grand mother

There is a story. My grandmother used to tell me. It’s a story of 100 year trust. It is a story how a mother sees her child growing day by day. And of course a story about the most trusted and oldest Ayurvedic product manufacturer company “DABUR”. As far I know for my grandmother Dabur and ayrvedic products are the same thing. So she always used to tell my mother not to use chemical products for me but use “Dabur”.  Only a mother can feel that satisfaction of using some safe and trusted product on her kid and just imagine the feel of happiness when a mother massage her kid’s soft body. Now let’s talk about “Dabur Lal tail” because this was the thing which connects me and my mother when I was just 1 or 2 year old. So we cannot ignore or avoid the contribution of this oil which helped to build such a beautiful and strong bond between me and my mother. My grandmother was like an instructor who had a long, good experience with this oil and always told everybody including my mother to use “Dabur Lal tail”. And of course she was my father’s mother and she was absolutely correct about this oil. I would like to tell few points about “Dabur Lal tail”. 1st is it is a fully aurvedic product so totally harmless. It does not affect or show any side effect on skin and a healthy massage with this oil strengthens the muscles. This is a much known product in my family and almost everybody using this oil for their child. So I just searched in google and came to know about its component. I would like to mention here
Til Tail (Sesame Oil) - helps in absorption
Ratanjyot
Shankhpushpi - protects against skin infection, general weakness
Camphor - stimulates blood circulation
Urad - nourishes muscles
Now let’s start my story. I am sure everybody knows Amitabh Bachhan. By birth I had some resemblance to him. He as well as I was suffering from a myasthenia Gravis. This is the muscle disorder which causes weakness of the muscles. It is due to some unknown change in genes. So initially nobody even doctors came to know about my disease. But within a few months this disease started showing its effect on me. Sometimes I used to get breathing attacks due to weakness in my breathing muscles. Sometimes I was unable to open my eyes due to weak eyelid muscles. My grandmother noticed when she saw that my one eye was open other was closed. This disease is very unique that it 1st affects eyelid muscles. I was just 6-7 month old at that time. My family immediately ran to the hospital. I went though few blood tests and diagnosed as myasthenia Gravis. My mother cried a lot but my grandmother was very firm and confident. Doctors told that as such there is no curative treatment but I can improve with physiotherapy, massage, yoga etc. And that was 1st day when this oil was used for my massage with strong and forceful suggestion of my grandmother. My grandmother never lost hope. My mother and grandmother continuously did massage for 1 year with “ Dabur Lal tail”. Slowly- slowly I improved infect I improved a lot. And the day came what for my entire family was dreaming. I walked 1st time. It was such a lovely, strong, intense, emotional moment for my mother and grandmother that everybody had tears in their eyes. This was the story my grandmother used to say to me whenever I sit with her and spend time.  Now I am totally cured and that credit goes to my mother, grandmother and of course “Dabur Lal tail”.

immune india

An autobiography of  fighter cell

I am a cell. My name is WBC means white blood corpuscle. I fight against disease, bacteria and virus. I save my land (body) at any cost. I stand in front of all enemies of body without thinking that I may die in this battle. I work hard I run from here to there in blood from head to toe. All my life I did struggle to keep this body disease free. My other friends like antibodies, antiviral cells and antibacterial cells are always with me to cope with these notorious bacteria and viruses. We work in a group and with effective strategy just like hunters kill their prey with a plan. We are the part of immunity. We are an immunity of the body and I promise that I will fight till the last breath.
That was a hot day. I could feel the silence before the storm. I could see my friends were waiting for some predictable disaster. Suddenly thousands of polio virus started coming in through elementary system. We were ready, but these viruses were very strong. They attacked on body with huge numbers. We fought alone. We did not have the extra support of polio vaccine. They were killing us brutally. We were dying here and there. Within few hours I could see dead bodies of my friends buried in the circulation. I was fighting bravely but in the absence of a vaccine it was almost impossible to win. I lived with self-respect and died with honor.
That is a small autobiography of Wbc. A fighter cell that saves the body from all disease. Today this killer cell lost the battle but we could have saved the cell if we had followed some universal instructions. Let’s see how we can make a healthy and immune India

Immunisation – vaccination

Each and every person in India should immunise their children. There is a time schedule of vaccine which should be strictly followed. The vaccine is the most important tool to make an Immune India. Nowadays science has developed vaccines against almost all severe disease like TB, Hepatitis B, diphtheria and other disease.

Nutrition

Yes, a rich nutritive food is very necessary to make the body healthy and disease free. Of course I can add Dabur Chaywanprash which provides 3 times more immunity that helps fight virus, flu and infections. So every Indian should be provided a healthy food to make entire India immune.

Yoga/ Sports/Exercise

Yoga is an ancient technique to keep the body strong and healthy. 10 min yoga everyday increases circulation in the body and makes the heart and mind strong. Yoga gives an internal strength to fight with bed ridden disease. Sports also work same as Yoga and help to make Immune India.

Hand wash

You might shock but unhygienic and unwashed hand has been proven as one of the most common cause death in children. It causes diarrhea and pneumonia which are the most fatal diseases for children in India. So proper hand wash should be kept in mind before having meal to make Immune India.

No to Tobacco and alcohol

Well I don’t think any explanation is required to prove that these addictions not only harmful for health but it destroy families and financial structure also. Let’s say No to all, these things to make entire India immune.

 Education

Finally education is the key to any troublesome lock. Our government and NGO sector should work enthusiastically and vigorously to educate and give proper information about all important aspects to achieve immunity. Let’s educate every Indian to make entire nation disease free.

It is next to impossible to include all the points in such a small article. But I want to conclude this with a beautiful Hindi poem.

Vaccine for Acne

Many of the current therapies focus on reducing the number of P. acnes organisms. and most recent developments have been in the use of combination therapies. Benzoyl peroxide and the retinoids have been added to topical antibiotics with the hope that the frequency of resistance will decrease, thereby improving the therapeutic outcome. On the other hand, only a few therapies focus primarily on reducing inflammation. Several previous studies have demonstrated that both cellular and humoral immunity against P. acnes are present in patients, and recent studies have provided a better understanding of the innate immune response produced by hosts in response to P. acnes. Upon activation, host innate cells elaborate a protective antimicrobial response to P. acnes by inducing the production of antimicrobial peptides such as human β-defensin 2. generated antibodies using inactivated P. acnes, postulating that they might provide protective immunity. intranasal immunization of mice with this inactivated vaccine generated in vivo protective immunity against P. acnes challenge. Specifically, the antibodies elicited by inactivated P. acnes attenuated IL-8 production in human sebocytes; however, there was no effect on P. acnes growth. The clinical improvement observed in this P. acnes inflammatory murine model suggests that for acne treatment, antibodies that primarily exhibit anti-inflammatory properties might be sufficient for clinical improvement but without an antimicrobial effect. The inactivated P. acnes vaccine used in this study targeted the whole bacterium, most likely without specificity. Therefore, developing vaccines against specific P. acnes antigens might be even more useful. study offer an important and interesting concept—that focusing on attenuating the inflammatory component of the disease could be therapeutically beneficial. Because the induction of cytokines, chemokines, and metalloproteinases by P. acnes occurs via a Toll-like receptor 2 (TLR2)-dependent pathway, the development of vaccines or other immune therapies that target TLR2 and other TLRs may provide other alternatives to conventional therapy.
